Powers/Abilities: He caused paranoid feelings in his victims. He had a cloak from which he could send out different creatures. Without his cloak he was stuck to a cross and used the chains hanging from it as his weapons.
(Venom: The Madness#2): Paranoia was ordered by Dusk to wait in the hypnagogic limbo for Eddie Brock and then lead him to Dusk. Paranoia attacked Eddie in his dreams with bats and nearly succeeded in pushing him into the portal to the Realm of Madness. The next time Paranoia used Venom's fear of Spider-Man against him and Venom woke up seconds before Necromancer could welcome him to the Realm of Madness.
(V:TM#3): Paranoia arrived on the scene shortly after Necromancer had gone. Venom was afraid that he was death, but Paranoia told him that Venom was only paranoid. Paranoia sent out his bats again and they attacked Venom together with tentacles coming out from Paranoia's cloak. Venom fought back when some additional claws grew out of his hand thanks to the Mercurial Virus. He attacked Paranoia, but when he sliced through the cloak it was empty and he wondered if Paranoia really existed.
(Nightmare#1-BTS): The cloak was gone and Paranoia became the partner of Phobia. They left the Realm of Madness and observed Nightmare in his own realm.
(Nightmare#1): Paranoia was nearer than ever to the sacred turf of Nightmare, who didn't seem at all bothered by this. He talked with Phobia about the problems of Nightmare and then looked in fear to the unborn Exile who came from the Realm of Exiles to Nightmare's realm. Paranoia together with Phobia started an attack on Nightmare's realm shortly after the master of the realm left for Earth. They defeated the Netherdemons and nearly claimed the portal. Suddenly the Exile awakened and went to the portal. Paranoia bowed to him and looked on as the Exile claimed the portal to the Dreamworld.
(NM#2): Nightmare saw Paranoia, Phobia and Doggerel in a dream when he tried to see what was going on in his realm. Paranoia was now an agent of Exile and he was afraid of his new master.
(NM#3): Paranoia was sent to Earth by Exile to assassinate Nightmare. He was thrown into the portal to the Dreamworld and entered the body of a girl that sat on Lucre's Nightmare Chair. Paranoia left Club Fear in the body of the girl and began his search for Nightmare. He found him in the evening strolling around in front of his new girlfriend's apartment. Paranoia attacked him with hooks, but Nightmare used his powers to defeat him. Paranoia was banished from the girl's mind and body. He returned to Nightmare's realm and the defeat made him more paranoid than ever before.
(NM#4): Paranoia became a loyal servant of Doggerel after the Exile was defeated. He bowed before his new leader who was only seconds later willingly replaced by Neurotica.

I think the cloak he possessed in his first appearance was similar to the cloak of Cloak (Tyrone Johnson).
Although the Paranoia from the Venom mini and the one from the Nightmare mini look different they are actually the same. They have the same smile (even the teeth are the same), name and place of origin. Just the cloak is missing and he put a mask on. The cloak was probably lost after the fight against Venom. No more explanations needed.
