History:
(Jay and Silent Bob Fight Back (movie within movie)) - Daredevil fought a group of ninjas in a city street.

Comments: Created by Kevin Smith, based on the character created by Stan Lee and Bill Everett.

To explain: In 2001's Jay and Silent Bob Strike Back movie there is a scene where the titular characters are being chased through the backlot of a Hollywood studio when they accidentally crash through a production filming what is clearly meant to be a Daredevil movie, complete with an uncredited actor or stuntman playing the costumed crimefighter. Obviously, the character in J&SBSB is meant to be someone playing Daredevil, and that J&SBSB character isn't Appendix worthy; however, going meta, since all fictions are considered to be a real reality somewhere in the multiverse, the Daredevil character within that fictional movie within the real movie is an actual DD counterpart, and can be covered. Or, to paraphrase Kirk Lazarus: "I know who can be profiled. Not the dude, nor the dude disguised as the dude, but the dude played by a dude disguised as another dude."

If anyone out there knows who was wearing the costume for the scene in question, please tell us; we'd love to give him due credit. Chronologically, he's the second out of four live action Daredevils to date, preceded by Rex Smith in Trial of the Incredible Hulk, and followed by Ben Affleck (who also appears in J&SBSB) in the 2003 Daredevil movie, and Charlie Cox in the 2015 Netflix series; he'd be third of five if we count Ben Carruthers, who seems to have been the preferred choice for the part had Angie Bowie's 1975 live action Daredevil and Black Widow series come to pass.

In Jay and Silent Bob Strike back the man playing the director of the Daredevil movie was Mark Steven Johnson. This proved prophetic, as two years after J&SBSB Johnson directed the real Daredevil movie, which starred Ben Affleck (as mentioned above) and included a cameo by Kevin Smith, who wrote, directed and starred in J&SBSB.

I'm ASSuming that the character shares the Earth-616 (mainstream Marvel universe) Daredevil's secret identity and basic background. We only see him for a few seconds, so of course nothing is confirmed, but since it was just a throwaway cameo I imagine Kevin Smith didn't have any changes in mind for the character's backstory.
