History:

(Free Comic Book Day 2011 (Thor the Mighty Avenger)#1 (fb) - BTS) - Captain America was used as a propaganda tool and occasional soldier by the U.S. military. He was assigned a mission against the Red Skull and given a symbol on his right palm.

(Free Comic Book Day 2011 (Thor the Mighty Avenger)#1) - In 1942, during a raid on the Red Skull's secret laboratory, Captain America was called forward by fellow US soldiers to open a reinforced door using his altered palm. The vault revealed a plethora of mystical artifacts and devices. Cap was drawn to a goblet that had secretly been enchanted by Merlin (who had been imprisoned by Loki) centuries before, calling for great warriors to free him. Cap touched the goblet and was instantly thrown back in time to King Arthur's realm alongside a similarly transported Thor. Although strangers to each other, they overcame hidden assassins on the road to Camelot. The two then met the killers' quarry: King Arthur, Sir Gawain and Merlin (actually Loki claiming to be the "regenerated" Merlin) and the jeweled religious artifact, the Holy Grail. Cap donned a spare armored chestplate. Loki distracted the others with illusory monsters while he made off with the Grail, but was halted by Captain America. Advised by Thor, Cap smashed Loki's belt, which contained Merlin's unbridled essence. Thus released, Merlin expelled the Grail through a dimensional portal, but Loki greedily followed it. Now freed, Merlin sent the two heroes back to their own times. Impressed by their valor, King Arthur had them memorialized in an artpiece. Now returned to the vault, Cap advised his fellow soldiers not to touch the goblet. Decades later, Thor's paramour, Jane Foster, found the artpiece. but dismissed any likenesses.

It's assumed that the Red Skull's secret base was somewhere in central Europe. Artifacts in the base included the dire Wand of Watoomb.

"Fighting American," the nickname provided by one soldier, likely refers to the character, the Fighting American, a similar, patriotically themed hero created by Joe Simon and Jack Kirby as a counter to Captain America. The Fighting American has been published by various companies, including Marvel.

I consider the FCBD 2011 issue part of Earth-10091 given the writer and artist, costumes and character context of Thor. It's also included in the compendium collecting all of this Thor the Mighty Avenger series.
