History:
(Daredevil I#265) - During the Inferno event, demonic energies caused inanimate objects to come to life, attacking civilians all over New York City. One dentist was operated on by his demonic machines, and they transformed him into a maniacal man who called himself the Dentist. Transformed, he told his secretary Rachel that he was taking the rest of the day off, and bid hello to patient Jack on his way out, both asking about each other's wives. After killing a few police, the Dentist put on some of their gear and attacked civilians. After terrorizing people for a time, the Dentist was confronted by Daredevil, who stabbed the creature through the center and then pulled its limbs off. Daredevil saw the city get back to normal, including civilians Charlie and Susan.

(X-Factor Annual I#4/3) - Government agents Jake Farber and Elwood McNulty interviewed Dr. Ned Zane, who claimed his dental equipment had corrupted him. Doctors took him away.
